Before commencing treatment, all clients are required to read and complete our Informed Consent Form. This process is designed to ensure you understand the services we provide, your rights and responsibilities, how your information will be managed, and what you can expect throughout your care.
We encourage you to read this document carefully. If there is anything you do not understand or would like to discuss, please let your clinician know before your first appointment begins. Your clinician will be happy to answer any questions and ensure you feel comfortable before providing consent.
Additional Consent for DBT Skills Groups
If you are participating in DBT Skills Groups only, you are also consenting to communication between DBT Clinics Australia’s DBT Skills Group facilitators and your nominated treating therapist, who may be external to DBT Clinics Australia.
This communication supports safe, coordinated, and effective treatment. In comprehensive DBT, the primary individual therapist is responsible for the overall clinical management of your treatment, including treatment planning, risk management, and decisions regarding access to other treatment modalities. Collaboration between your individual therapist and DBT Skills Group facilitators helps maintain treatment fidelity and ensures that your care is integrated and consistent.
Where you are participating in a comprehensive DBT program through DBT Clinics Australia, this consent is typically discussed and obtained during the orientation and pre-treatment phase before you commence DBT Skills Groups.
By signing the Informed Consent Form, you acknowledge that you have read and understood the information provided, have had the opportunity to ask questions, and voluntarily consent to receiving services from DBT Clinics Australia.
