Coordinated care is an important part of effective mental health treatment. This form allows you to give DBT Clinics Australia permission to securely exchange relevant clinical information with another health professional, service, or organisation involved in your care.
You remain in control of what information can be shared and the purpose for which it may be used. You can specify the records you authorise us to exchange, such as assessments, reports, treatment summaries, progress information, or other relevant clinical records.
Providing consent is entirely voluntary, and you may withdraw your authorisation at any time, except where information has already been released or exchanged in accordance with your consent. Unless withdrawn earlier or its purpose has been fulfilled, the authorisation remains valid for 12 months from the date it is signed.
If you have any questions about sharing your information, please speak with your clinician or contact our team before completing the form.
